House of Care is a small personal care home for those who have declining or compromised health and who lack the housing, financial or family resources to receive the assistance they need with activities of daily living. While we accept residents with a range of physical health conditions, we specialize in helping those with limited-life expectancies.*
The House of Care seeks to provide a facility complete with:
Private rooms for 5 persons.
24-hour onsite staff.
Congregate dining and living areas.
3 daily meals for residents.
Personal laundry service.
Time and space for loved ones.
Caring, trained staff and volunteers.
Individual case management.
Support services for loved ones.
The warmth and security of a home.
Residency requirements: Low to moderate income residents of State College and Centre County receive preference. Residents must be at least 18 years of age.
For those receiving end of life care, treatment goals are directed toward relief of symptoms and may include curative treatments of the underlying disease. We believe that care received at the end of life should not only be high quality but also respectful of the desire for peace and dignity.
As appropriate, referrals for nursing care are made to community Home Health Care services and Hospice Programs.
*Limited life expectancies means that life is measured in months rather than years. A life-limiting condition may be due to a single diagnosis, a combination of diseases or no specific diagnosis. |
